Background

  • Patient with dual chamber pacemaker (DDD 60-130) implanted for syncope and complete heart block

  • Irregular heart rate (ECG below)

There are pacing spikes followed by broad ventricular beats (ventricular capture)

​

There are both intrinsic p waves and pacing spikes followed by a p wave.

​

After each atrial event is a ventricular paced beat. Appears to be consistent with DDD mode and atrium is being sensed appropriately.

​

There are variations in the PR interval. Beats 1 & 7-12 have PR

~200ms, where as beats 2-6 have a prolonged PR interval ~400ms.

​

This is consistent with an AV search algorithm whic was subsequently turned off due to permanent AV block
