The pacing lead is implanted at the area of the bundle of HIS.
The EGM should have a clear HIS signal followed by an RV signal ~40-55ms after the HIS signal
Pacing at the HIS region can result in 3 possible outcomes
•Selective/pure HIS capture
•Non selective HIS capture (HIS and RV both captured)
•RV only capture (no HIS capture- should move lead)
Location of capture can be determined by the 12 lead ECG

Limitations/Complications
Can have sudden high thresholds
Can have very low R wave amplitude (sensing issues)
Can potentially develop distal conduction block resulting in sustained pauses
